Location:         Claveria Police Station (Centro 1, Claveria, Cagayan)

REQUIREMENTS:

To successfully apply for the National Police Clearance, kindly bring the following documents:

One (1) Valid Identification Card (ID) – Acceptable IDs include (but are not limited to):

• Passport
• Driver’s License
• Postal ID
• Voter’s ID
• PhilHealth ID
• Unified Multi-Purpose ID (UMID)

APPLICATION PROCESS:

1. Walk-in during the specified time to the Claveria Police Station.
2. Proceed to the designated application area, where our helpful staff will guide you through the necessary forms and procedures.
3. Bring your one valid ID for verification.
4. Pay the corresponding processing fee (if applicable).
5. Your application will be processed, and the National Police Clearance will be issued to you upon approval.

IMPORTANT NOTES:

1. Ensure that all information provided in the application form is accurate and up-to-date for smooth processing. In absence of 1 valid ID, secure BARANGAY CERTIFICATION as a proof of identity.
